Q. What is the i'rab of الكريم in the sentence جاء محمد الكريم
It is marfoo' based on being either a Sifah or Badal. 
Q. ​when illustrating the instances when a nakirah can occur as a mubtadat' the above cases were mentioned. They are not very clear to me. Could a brief explanation be provided please?

 ـ أو يعطف عليها بمعرفة . نحو : رجل ويوسف في المنزل 

 ـ أن يعطف عليها بنكرة مخصصة . نحو : رجل وامرأة طويلة واقفان 

 ـ أو تعطف على نكرة موصوفة . نحو : تميمي ورجل في المنزل

In examples 1-3 the indefinite mubtada is ma'toof to an ism which has the right to be a mubtada itself ( ism alam, an indefinite word that is mowsoof). In this case the atf makes it permissible to start the sentence off with an indefinite word.
 
In example 4 it starts off with an indefinite word that is mowsoof.  An indefinite ism can be a mubtada when a sifah is brought for it
